Small group living with maximum 2 young people

SEMH/EBD (Social, Emotional, Mental Health & Emotional & Behavioural Difficulties)

Emergency, Short-term, and Long-term placements
Our Aims
Work within existing Care Plans to support individual needs and goals
Rehabilitate young people back to their families or prepare for independent living
Provide assessment and support for educational and social development
Promote education, life skills, and positive relationships

Contact our team to discuss the young person's needs and our capacity to provide appropriate care.
Pre-Admission Meeting
Comprehensive assessment meeting with the young person, family, and relevant professionals.
Placement Planning
Development of individualized care plan and transition arrangements for the young person.
Review & Support
Ongoing monitoring, review meetings, and continuous support for the young person and family.
Emergency Referrals
We understand that sometimes young people need immediate support. Our emergency referral process is designed to provide rapid assessment and placement when urgent care is required.
